APPOINTMENT CANCELLATIONS AND RETAIL POLICIES
- Cancellations with between 12-24 hours notice will be charged 50% of the service price.
- Cancellations with less than 12 hours notice will be charged 100% of the service price.
- All “no-shows” to scheduled appointments will be charged 100% of the service price.
- Late cancellations and no-shows to insurance claim-based appointments will be charged the cancellation fee according to the regular cash-based rate.
- If we are able to fill the late-canceled appointment, you will not be charged the cancellation fee.
Late Arrivals
Out of respect and consideration to your practitioner and other patients and clients, please plan accordingly and be on time. Arriving late, may result in a shortened session in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your practitioner will determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the cost of the session booked.
Product Returns
All retail products (herbs, supplements, physical therapy tools, books, etc) can be returned for a full refund within 30 days, with proof of purchase, as long as the product is unused and unopened, and has not been damaged in any way.
Treatment Packages
All packages come in quantities of 3, 5, and 10 sessions, are non-refundable, and can be shared.
GUIDELINES REGARDING ILLNESSES
Shen Shen Health and Harmony has put preventative measures in place to reduce the spread of illnesses. These measures include, but are not limited to: air purifiers located in every treatment room and in common areas; staff and patients are encouraged to stay home should they feel ill or exhibit symptoms of illness; staff and patients are encouraged to wear masks in the clinic if there is any doubt about personal health.
MASKS ARE OPTIONAL IN THE CLINIC
Staff and patients should exercise their best judgment regarding masks — if you have been exposed to an illness, but are asymptomatic, you are encouraged to wear a mask while in the clinic to mitigate the spread of any illness. Staff will wear a mask while in session upon request.
At minimum, Shen Shen Health and Harmony follows current CDC guidelines regarding illnesses. In some cases, staff and patients may be asked to be more cautious than the CDC recommends.
NOTICE OF PATIENT PRIVACY (HIPAA)
I understand that as part of my healthcare, Shen Shen Health & Harmony originates and maintains health records describing my health history, symptoms, examination and test results, diagnoses, treatment and any plans for future care of treatment. Any information that I choose to share with Shen Shen Health & Harmony, including my admission as a patient of this facility, will be kept confidential, used as specified below and shared only with my written consent.
I understand that this information serves as:
- A basis for planning my care and treatment.
- A means of communication among the many healthcare professionals who contribute to my care.
- A source of information for applying my diagnosis and surgical information to my bill.
- A means by which a third-party payer can verify that services billed were actually provided.
- A tool for routine healthcare operations such as assessing care quality and reviewing the competence of healthcare professionals.
I understand that I have the right:
- To object to the use of my health information for directory purposes.
- To request restrictions as to how my health information may be used or disclosed to carry out treatment, payment or healthcare operations – and that the organization is not required to agree to the restrictions requested.
- To revoke this consent in writing, except to the extent that the organization has already taken action in reliance thereupon. I will notify Shen Shen in writing if I require information to be shared with anyone other than those directly involved with my care at Shen Shen Health & Harmony.
